NOTICE OF PROPOSED CLASS ACTION SETTLEMENT

THIS IS A COURT-AUTHORIZED NOTICE. IT IS NOT A NOTICE OF A LAWSUIT AGAINST YOU OR A SOLICITATION FROM A LAWYER.

All individuals, including, but not limited to, patients and potential patients to whom Lifeworks Wellness Center, LLC (“Lifeworks”) sent written notice from Lifeworks regarding the Data Breach.

A proposed Settlement has been reached in the class action lawsuit Tanya Fabregas v. Lifeworks Wellness Center, LLC, Case No. 23-CA-014579 in the Thirteenth Judicial Circuit in and for Hillsborough County, Florida (the “Lawsuit”).
The Lawsuit asserted claims against Lifeworks arising out of the Incident.

If you are a member of the Settlement Class, you have the following options:

 

YOUR LEGAL RIGHTS AND OPTIONS IN THIS SETTLEMENT
SUBMIT A CLAIM BY SEPTEMBER 7, 2024	You must submit a valid Claim Form to receive credit-monitoring services from the Settlement and reimbursement for unreimbursed expenses and losses.
DO NOTHING	You will receive no benefits from the Settlement and will no longer be able to sue the Released Parties, including Lifeworks, over the claims resolved in the Settlement.
EXCLUDE YOURSELF FROM THE SETTLEMENT BY JULY 9, 2024	You will receive no benefits from the Settlement, but you will retain your legal claims against the Released Parties.
OBJECT BY JULY 9, 2024	Write to the Court about why you do not like the Settlement. You must remain in the Settlement Class to object to the Settlement.
